What is a Bariatric Psychological Evaluation?

A bariatric psychological evaluation is a clinical assessment designed specifically for individuals considering bariatric surgery (gastric bypass, gastric sleeve, LAP-BAND, etc.). The evaluation determines your psychological readiness for surgery and identifies support you may need.

The evaluation includes:

  • Clinical interview — Your motivation for surgery, understanding of the procedure, and expectations

  • Mental health assessment — Screening for depression, anxiety, eating disorders, substance use, and other conditions

  • Psychological readiness assessment — Your ability to make lifestyle changes, follow post-operative requirements, and manage emotional challenges

  • Support system evaluation — Family support, coping skills, and resources for success

  • Written report & clearance — Clinical findings and recommendation for surgical clearance

What to Expect

The Process

1. Pre-Evaluation You'll provide information about your surgery plan, medical history, and why you're pursuing surgery.

2. Clinical Interview (45–55 minutes) We'll discuss:

  • Your weight history and previous attempts at weight loss

  • Your motivation and expectations for surgery

  • Understanding of the surgery and post-operative lifestyle requirements

  • Mental health history (depression, anxiety, eating disorders, trauma)

  • Substance use history

  • Family and social support

  • Coping skills and stress management

  • Realistic understanding of outcomes

3. Psychological Assessment We screen for:

  • Depression and anxiety

  • Eating disorders or disordered eating patterns

  • Substance use concerns

  • Personality factors that affect surgery success

  • Trauma or unprocessed grief that could impact recovery

4. Written Report & Clearance (within 3–5 business days) Your surgical team will receive:

  • Psychological assessment findings

  • Readiness determination

  • Recommendations for pre- or post-operative support

  • Clearance status (cleared, cleared with conditions, or recommendation for additional counseling)

Bariatric Psychological Evaluation (55 minutes): $150

Turnaround Time: 3–5 business days for completed report

This is a self-pay service. We do not bill insurance for evaluations, but you may be able to submit the report to your insurance for reimbursement (check with your carrier).

What Makes Someone a Good Candidate?

Good indicators:

What Happens After Clearance?

If you're cleared:

  • Move forward with your surgical team's timeline

  • Consider post-operative counseling to support lifestyle changes

  • Join support groups for bariatric patients

  • Plan for post-operative psychological support if needed

If you're cleared with conditions:

  • We'll recommend specific support (therapy, counseling, support groups)

  • You complete these before surgery

  • We provide updated clearance once conditions are addressed

If additional counseling is recommended:

  • We can provide therapy or refer you to a therapist

  • This isn't about blocking surgery — it's about setting you up for success

  • Many people benefit from counseling before AND after surgery

Common Questions

Q: Will this delay my surgery?
A: Generally no. Most evaluations are completed within 3–5 business days. Your surgical team can plan around this timeline.

Q: What if I don't get cleared?
A: Clearance depends on what we find. If there are concerns, we discuss options (counseling, therapy, support groups) to address them. Most people do get cleared once concerns are addressed.

Q: Will you tell my surgeon if I have depression or anxiety?
A: Yes. Your surgeon needs to know about mental health conditions so they can provide appropriate support. This is standard practice and helps ensure your success.

Q: What if I'm nervous about the surgery?
A: That's completely normal! Being nervous is different from not being ready. We assess your overall readiness, coping skills, and support system — not whether you're anxious.

Q: Can I bring someone to the evaluation?
A: Generally, we recommend coming alone so you can speak freely. However, if you have accessibility needs, we can discuss arrangements.

Q: What if I haven't decided on a surgeon yet?
A: That's fine. You can get evaluated to understand your readiness and what support you might need, then move forward when you're ready.

Q: Do you do post-operative support?
A: Yes. After surgery, many people benefit from counseling to navigate lifestyle changes, emotional challenges, or body image concerns. We offer therapy separately from the evaluation.
