At Serenity Space, we conduct detailed disability and FMLA evaluations that clearly document your condition, functional limitations, and need for accommodations or leave.
What is a Disability/FMLA Evaluation?
A disability/FMLA evaluation is a comprehensive clinical assessment that documents:
Your diagnosis — Mental health condition(s) affecting your ability to work
Functional limitations — What you cannot do due to your condition
Medical necessity — Why accommodations, modified work, or leave is medically required
Duration — How long accommodations or leave are expected to be necessary
Clinical recommendations — What modifications would support your functioning at work
This documentation is submitted to your employer's HR department, disability insurer, or FMLA administrator.

What to Expect
The Process
1. Pre-Evaluation You'll provide information about your condition, how it affects your work, and what accommodations or leave you need.
2. Clinical Assessment (90 minutes) We'll evaluate:
Your current symptoms and how they impact daily functioning
How your condition affects your ability to perform work tasks
Previous treatment, medications, and what's been tried
Your work history and current job demands
Impact on concentration, reliability, memory, emotional regulation, etc.
Expected timeline for recovery or improvement
3. Detailed Clinical Report (within 3–5 business days) Your employer/benefits provider will receive:
Diagnostic summary
Functional limitations (specific to work)
Medical necessity for accommodations or leave
Recommended modifications (timeline, specific needs)
Return-to-work timeline (if applicable)
Professional recommendation

Disability/FMLA Clinical Assessment (90 minutes): $450
Turnaround Time: 3–5 business days for completed report
This is a self-pay service. We do not bill insurance for evaluations, but you may be able to submit for reimbursement (check with your carrier).

Common Questions
Q: Will this hurt my job security?
A: No. Requesting accommodations under ADA is a legal right. Employers cannot discriminate or retaliate for requesting accommodations. The report is confidential between you and HR.
Q: What if my employer says no?
A: If your employer denies reasonable accommodations despite medical necessity, you may have legal recourse. We recommend consulting an employment lawyer. Our report provides the clinical documentation supporting your need.
Q: How long will accommodations be in place?
A: That depends on your condition and treatment. We'll estimate timeline based on typical recovery. As you improve, accommodations can be adjusted.
Q: What if I need extended leave?
A: If FMLA leave is medically necessary, we document that. FMLA allows up to 12 weeks of unpaid leave per year. We certify the medical necessity.
Q: Will my HR department contact you?
A: Possibly. HR may send a certification form asking us to verify information. You'll sign a release, and we'll respond directly to HR.
Q: Can I use this for disability benefits?
A: Yes. This assessment can support an application for Social Security Disability (SSDI), short-term disability, long-term disability, or workers' compensation, depending on your situation.
Q: I'm worried my employer will see this as negative.
A: Requesting accommodations shows you're proactive about your health. Many employers appreciate employees who take care of themselves — it reduces overall absences and improves performance.
Q: What if I've been in therapy with someone else?
A: That's fine. We can review those records (with your permission) and provide an independent clinical assessment. We work with your existing treatment team.
